Prior to the first day of practice, all golfers must complete the following requirements.

• Have a current physical on-file with the Davies Activities Office/Athletic Trainer.

• Complete required athletic forms via PowerSchool registration:

  • Co-Curricular Student/Parent Meeting

  • Concussion Management

  • HIPAA

  • Consent for Treatment

• Participate in one 18-hole tournament or two 9-hole tournaments OR shoot 60 (or lower) on 9 holes at qualifying on Monday August 10.
