Co-op/Home School Students

To be completed by all Non-FPS and home school students who are participating in FPS Activities.  All information will be sent to the appropriate school to be processed. 

If you are an FPS student, please complete these items within the Registration Forms on the parent's Powerschool account.

Section 1: Sign consent on the Concussion Management information and protocol verification, and the authorization for release of medical information form (HIPAA) *Required

Section 2: Online Student-Parent Activity Video.  In place of attending a meeting at a Fargo Public School you can watch and verify your consent in Section 2.  *Required (either watching the video or attending a meeting on campus)

Section 3: Email a copy of your child’s physical which will be sent to the appropriate school. *Required (if you cannot upload your physical please bring a copy into the school at which your child is participating)

“Prior to participation (including practice), a student participating in NDHSAA sanctioned athletic activities and representing his/her school must have on file with the superintendent, principal, athletic director or school nurse, an annual NDHSAA-approved Athletic Pre-Participation Health History Screening and Physical Examination clearance form completed by a qualified health care professional (i.e. Doctor of Medicine of Osteopathy, Nurse Practitioner or Physician Assistant under the supervision of a physician); the Athletic Pre-Participation Health History Screening and Physical Examination is valid for one school year; a physical examination completed before April 15 is not valid for participation in the following school year.” – www.ndhsaa.com/about/forms
