NEW CONCUSSION MANAGEMENT POLICY

The North Dakota legislature approved a law, NDCC 15.1-18.2 that requires school districts to implement a concussion management program.  If your son/daughter plans to participate in any FPS activity, you will need to read the new Concussion Information Brochure and submit a Concussion Verification Form to the coach or athletic trainer PRIOR to the first day of practice. 

Both the Concussion Verification Form and the annual Physical Form need to be submitted or the student will not be able to participate.
